Vestibular migraines are giddiness caused by a migrainous problem. Now all these people who have giddiness which comes on suddenly it is a unilateral kind of a giddiness, dragging them to one side or causing a reeling sensation because of imbalance. This is caused by a history of migraine which is inducing the giddiness as well. It has been seen in people who have had a history of migraine. So it could be because of a neurological disorder or it could also be sometimes because of a vascular problem. Now we know that migraine is caused because of a narrowing of blood vessels temporarily causing a starvation of blood flow into that part of the brain causing the headache. The headache is basically caused because of increase in pressure within the skull in that particular area which is why they have a pressure and headache. Likewise there can be this similar system happening within the vestibular system. The vestibular system inside the inner ear maintains the balance in the body. Now these people can also complain of tinnitus or loss of hearing fullness in the ear because the balance organ system is connected to the hearing system, which is the cochlea. They are like siamese twins joined at the hip and as a result of that you can have one causing an impact on the other. So migraine in this case is the positive factor and that being the case we have to treat the patient accordingly with something to sedate the vestibular system reducing the symptoms which are there. Since we are not pretty sure about what is causing the problem we will have to treat it symptomatically and the surrounding factors that may be associated with the migraine. So the entire ecosystem of the migraine has to be handled accordingly.
